Hale/P200 Telescope Cover Sheet – Semester

Context: This Hale/P200 Telescope Cover Sheet shall be used by proposers from all P200 constituencies requesting classically-scheduled Hale Telescope observations. Some aspects of this form relate only to Caltech proposers. This form is designed exclusively for use on desktop computers.

Instructions: Fill in the required information. When finished, save the form on your computer as a PDF document (press the PRINT button to open the print dialog box on your computer, then save the form as a PDF) to be merged with the cover sheet and the discussion section of your proposal.
