SN 2004em:

This SN was discovered by Mark Armstrong (IAUC 8406) on September 14, 2004. Filippenko et al. (IAUC 8411) find it is a type II event, shortly after explosion.

The host galaxy IC 1303 is at z=0.014894 (NED).
